Strategy 3.1.2 – Support promising approaches to improving the delivery of comprehensive senior care within California
The Center for Technology and Aging (CTA) is a Center of Excellence with the aim to identify and rapidly diffuse technologies that hold the promise of improving the quality and affordability of care for seniors and helping those with chronic health care conditions to maintain their independence as long as possible. Through the 22 projects supported by the Center, CTA is specifically exploring scaling proven technologies in Medication Optimization, Remote Patient Monitoring, Care Transitions, and through Mobile Health applications (mHealth).

GIA is partnering with the American Society on Aging as its technical partner in their monthly webinar series, which will focus on Older Americans Act (OAA) reauthorization. Kathy Greenlee, the Assistant Secretary for Aging, will speak about the OAA and the challenges of reauthorization.
 
 

Bringing PACE to Humboldt County

Strategy 3.1.1 – Through collaboration with other funders, extend the work of national demonstration projects to California and develop in-state demonstration programs
In 2009 The SCAN Foundation provided a grant to On Lok supporting a year-long effort to evaluate the readiness of three rural counties in California to pursue Program for All-Inclusive Care for the Elderly (PACE), one of which was Humboldt. As a result of this project the Humboldt Senior Resource Center (HSRC) has begun the process of establishing a PACE program in Humboldt County. The funding for this project, in partnership with several other local foundations will support the development of HSRC's PACE application and licensing.
