The SCAN Foundation is an independent, charitable foundation whose mission is to advance the development of a sustainable continuum of quality care for seniors. We envision a society where seniors receive medical treatment and human services that are integrated in the setting most appropriate to their needs and with the greatest likelihood of a healthy, independent life.  We do this by providing grant support to activities that raise awareness and engage the public in this important dialogue, support research that provides insight into realistic policy solutions, and build upon promising programs that bridge medical care with supportive services for seniors in ways that respect their individual needs and value their dignity and independence.
