Funding Priority #2:  Advance realistic policy options to establish and finance a comprehensive continuum of care for seniors.

A genuine opportunity exists to reform the continuum of care.  Baby boomers caring for their aging parents and relatives often struggle to find support for themselves. Seniors and family members can become overwhelmed when trying to navigate a fragmented delivery system that generally favors acute and institutional care over chronic care and community-based long-term services and supports (LTSS).  The United States is in great need of new policy solutions aimed at improving service delivery and financing of LTSS as well as bridging the divide between medical and human services.

Policy makers at the state and federal levels need to be able to act upon concrete, realistic policy options.  The SCAN Foundation seeks projects that will lead to the advancement of realistic policy options that establish and enhance a comprehensive continuum of care for seniors; help to improve the effectiveness, efficiency, and availability of current LTSS and forge a consensus for long-term care policy reform.
