The SCAN Foundation invites the submission of Letters of Interest (LOI) from eligible organizations in support of its third funding priority:

Funding Priority #3: Support the dissemination and assessment of promising new program models that inform and strengthen long-term care policy development.
 
Strategy 3.1.2: Support promising approaches to improving the delivery of comprehensive long-term care within California.
 
By supporting the assessment and dissemination of promising models of long-term services and supports (LTSS), The SCAN Foundation contributes to improving the delivery and outcome of care for seniors. The results of successful model projects will inform the Foundation’s work on advancing realistic policy options (Funding Priority #2). The Foundation’s support for projects under this priority will be focused primarily in California.
 
The Foundation will also consider funding comprehensive evaluations of tested models, which previously had only limited evaluation efforts. Evaluations that measure, quality of care and quality of life for seniors, and feasibility and replicability of long-term service and support models will receive the strongest consideration.

The SCAN Foundation has adopted a rolling application process for its 3rd Funding Priority Letters of Interest (LOIs) are being accepted via the Foundation’s web-based grants management system on an ongoing basis. The following is a schedule of acceptance and review of LOIs and full proposals.

The criteria by which LOIs will be reviewed include:
 
  • Demonstrated understanding of LTSS issues and the needs of seniors.
  • Strength of proposed activities and their impact on our mission.
  • Commitment of organization, as demonstrated through previous efforts in this area.
  • A clearly articulated work plan with measurable and achievable results.
  • Demonstration of how proposed project supports routinizing or bringing to scale tested models or evaluating models with a clear sustainability plan.
  • Extent to which the work has an impact on or is relevant to California.

 

 The LOI application requests information about the organization and project, including:
 
  • Budget and budget narrative to be uploaded (maximum funding requested not to exceed $100,000
  • Duration of project (projects are typically 12 months or less, but should not exceed 18 months

 

A project narrative, 2-5 pages in length, should include:
 
  • Description of the project and its rationale
  • Statement of goals and objectives
  • Description of population served
  • Description of expected deliverables
  • Evaluation activities and expected outcomes
  • History of organization’s capacity to complete the project
