Innovation in Health Care Award

 
The SCAN Foundation invites California health care organizations and other entities working in partnership with a health care organization to submit applications for the second annual Innovation in Health Care Award. 
 
The theme of the 2018 award is Bringing the Essential Attributes of Quality Care to Life. 
 
Qualified submissions will demonstrate successful implementation of at least one Essential Attribute of a high-quality delivery system. Watch Next Avenue’s video, based on the Essential Attributes. 
 
The deadline for submission is June 19, 2018. Learn more below, and attend our annual California Summit on Long-Term Services and Supports where the awardee will be recognized.
