Strategic Plan

The SCAN Foundation is working to promote aging with dignity and independence for America’s growing number of seniors by advancing the development of integrated health care and supportive services that respect individual choices, foster independent living, and allow people to receive care where it is the most appropriate for them.

We believe that right now is the perfect time to transform the system of care for older adults. That is because baby boomers and their aging parents who are facing the stresses and challenges of the current care system have the power to move mountains if they work together for positive change. With that in mind, we seek to engage the public and policy makers to make this a priority, develop strong policy solutions that will help solve our challenges, and support promising programs that are changing health care for the better.

We’re accomplishing the goals of our first five-year plan by engaging the public, supporting sound policy solutions, and funding promising programs and studies from organizations in California and across the nation who share our desire to create a sustainable, connected system of care that places people first. This will help us keep our goals grounded in reality, by supporting what works. All of our funding decisions are guided by a set of priorities to ensure consistency and focus.
