These caregivers are taking on more complex and intensive responsibilities, facing emotional strain, financial stress, and social isolation. My mom passed away a\u00a0couple\u00a0years\u00a0ago.\u00a0He's\u00a090, hard hearing, little dementia going on. I went ahead and had him move in with me so I\u00a0can\u00a0take care of him. He doesn't\u00a0wanna\u00a0go into a nursing home, and I don't want to see him go into a nursing home. He has five kids.\u00a0He's\u00a0been a farmer. My job is to take care of him.\u00a0<\/span>I\u00a0can't\u00a0go and get\u00a0a job, but he\u00a0doesn't\u00a0make enough money to pay for everything and hire somebody to\u00a0come and take\u00a0care of him....\u201d\r\n\r\n<span data-contrast=\"auto\">What is accurately\u00a0being\u00a0described as a \u201c<\/span><a href=\"https:\/\/pmc.ncbi.nlm.nih.gov\/articles\/PMC7644552\/\"><span data-contrast=\"none\">caregiving burden<\/span><\/a><span data-contrast=\"auto\">\u201d\u00a0in the United States\u00a0is a symptom of a\u00a0deeper structural weakness\u00a0in how we care for older adults and people with disabilities.\u00a0Caregiving is\u00a0core\u00a0to the\u00a0social\u00a0and economic infrastructure\u2014no less essential than healthcare, transportation, or education\u2014yet it\u00a0remains\u00a0largely invisible, propping up a system that would otherwise falter.\u00a0<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Reliable and affordable care support\u00a0should\u00a0not\u00a0be viewed as a luxury when\u00a0it\u00a0is a necessity; expecting families to shoulder caregiving alone, without adequate public\u00a0assistance, places an unreasonable and avoidable burden on them. And\u00a0families are often relied upon to serve as caregivers because\u00a0of the twin challenges of\u00a0exorbitantly expensive out-of-pocket in-home care, coupled with a dire shortage of the paid care workers who\u00a0provide this support.\u00a0\u00a0<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">These gaps are experienced unevenly.\u00a0<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559991&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Black and Hispanic older adults who need LTSS are one-and-half times more likely to be without corresponding care support than older white adults, yet are less likely to receive paid help, instead relying heavily on family and friends.\u00a0<\/span><span data-contrast=\"auto\">These patterns reflect a system shaped by structural inequities from its\u00a0inception\u00a0where policy choices have long excluded and undervalued certain communities.<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559991&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">While\u00a0<\/span><a href=\"https:\/\/news.umich.edu\/most-older-adults-want-to-age-in-place-but-many-havent-taken-steps-to-help-them-do-so\/\"><span data-contrast=\"none\">polling<\/span><\/a><span data-contrast=\"auto\">\u00a0suggests that 88% of older adults aged 50-80 feel that it is important to remain in their homes for as long as possible, a\u00a0<\/span><a href=\"https:\/\/www.longtermcarepoll.org\/have-older-americans-done-any-planning-for-their-long-term-care-needs\/\"><span data-contrast=\"none\">majority have not begun<\/span><\/a><span data-contrast=\"auto\"> planning for their long-term care needs. For most Americans, comprehensive services to enable individuals to age in place are simply unaffordable. In 2022,\u00a0<\/span><a href=\"https:\/\/atiadvisory.com\/ltss-users-by-state-and-county\"><span data-contrast=\"none\">7.2 million individuals\u00a0relied on Medicaid-funded LTSS<\/span><\/a><span data-contrast=\"auto\">, including 6 million who received care through\u00a0home-\u00a0and\u00a0community-based\u00a0services (HCBS)\u00a0and 1.5 million who used institutional\u00a0supports.\u00a0The program is so popular that\u00a0more than\u00a0<\/span><a href=\"https:\/\/www.kff.org\/medicaid\/a-look-at-waiting-lists-for-medicaid-home-and-community-based-services-from-2016-to-2025\/\"><span data-contrast=\"none\">600,000 eligible people\u00a0are currently on waitlists<\/span><\/a><span data-contrast=\"auto\">\u00a0for HCBS.\u00a0These are\u00a0individuals\u00a0who qualify\u00a0for support, but\u00a0whose needs are not being met as\u00a0states\u00a0lack the capacity and funding.<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Even\u00a0though every state\u00a0has a Medicaid HCBS\u00a0program,\u00a0the majority of\u00a0older adults aren\u2019t\u00a0eligible for those services.\u00a0They make too much to qualify\u00a0for the public program,\u00a0but\u00a0not enough to afford it on their own. NORC at the University of Chicago\u00a0found that\u00a0\u201cthere will be\u00a0<\/span><a href=\"https:\/\/www.norc.org\/research\/projects\/the-forgotten-middle-in-2033.html\"><span data-contrast=\"none\">16 million middle-income seniors in 2033<\/span><\/a><span data-contrast=\"auto\">.\u201d\u00a0This\u00a0\u201cforgotten middle,\u201d\u00a0and their family caregivers,\u00a0will continue to struggle if we\u00a0don\u2019t\u00a0change how we\u00a0pay for\u00a0LTSS in\u00a0the US.\u00a0<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Recent news\u00a0articles suggest that the cost of in-home care, which is already out-of-reach for many Americans, is\u00a0<\/span><a href=\"https:\/\/www.axios.com\/2025\/10\/30\/trump-immigration-elder-care\"><span data-contrast=\"none\">rising more than three times faster than inflation<\/span><\/a><span data-contrast=\"auto\">. This fact, in addition to\u00a0recent federal\u00a0Medicaid cuts,\u00a0and an ever-expanding caregiver shortage,\u00a0has left us with\u00a0a long-term care system that is in a\u00a0<\/span><a href=\"https:\/\/www.washingtonpost.com\/business\/2025\/12\/04\/elder-care-home-health-shortage\"><span data-contrast=\"none\">deepening crisis<\/span><\/a><span data-contrast=\"auto\">.<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Reimagining the system is\u00a0necessary and\u00a0possible.<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Centering the voices of older adults and family caregivers must be foundational to any meaningful reform. Efforts like The People Say show that caregivers and older adults offer clear, consistent insight into where systems fall\u00a0short\u00a0and what meaningful support looks like in practice.\u00a0<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Treating these insights as decision-shaping evidence grounds policy in how the system\u00a0actually operates\u00a0in people\u2019s lives, sharpening the ability to\u00a0identify\u00a0structural gaps and build a more accountable and\u00a0equitable\u00a0LTSS system.<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<b><span data-contrast=\"auto\">Roadmap for Action<\/span><\/b><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">2026 presents\u00a0a timely\u00a0opportunity for\u00a0policymakers\u00a0to reassess and address\u00a0these care\u00a0challenges\u00a0in a significant way.\u00a0In the last few years,\u00a0the\u00a0public\u2019s\u00a0awareness of caregiver challenges\u00a0have\u00a0risen\u00a0sharply\u00a0and calls for real change are\u00a0<\/span><a href=\"https:\/\/www.npr.org\/2025\/12\/30\/nx-s1-5660935\/family-caregiver-support-policies-states\"><span data-contrast=\"none\">growing louder<\/span><\/a><span data-contrast=\"auto\">.\u00a0During the 2024 presidential election, candidates\u00a0from both parties\u00a0put forward specific policy proposals to support\u00a0caregivers and bolster in-home care, an unprecedented political development that\u00a0in 2026,\u00a0is\u00a0continuing to drive\u00a0dialogue and action\u00a0to move\u00a0these issues forward\u00a0at the federal level.\u00a0Likewise,\u00a0at the state level,\u00a0policymakers\u00a0are advancing programs that\u00a0help\u00a0caregivers and their loved ones,\u00a0such as paid\u00a0family and medical leave\u00a0and\u00a0tax credits\u00a0to help offset care costs.<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Now is the time to\u00a0leverage\u00a0and build on this momentum.\u00a0The good news for policy leaders\u00a0is that\u00a0there is no need\u00a0to\u00a0start from scratch\u00a0as\u00a0there\u00a0are\u00a0already\u00a0well-established\u00a0roadmaps\u00a0for action.<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Medicaid plays\u00a0a central role\u00a0in supporting HCBS, but it is an optional benefit that varies widely by state in scope and is available only to those with the lowest incomes. As a result, access to care depends more on geography and poverty status than on need. Public support for caregiving should be a standard feature of our care infrastructure, not a patchwork safety net that families must fight to access.<\/span><span data-ccp-props=\"{&quot;335559685&quot;:360,&quot;335559731&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Importantly, Medicaid is not the only framework through which Congress can act. home-based services.\u00a0Taken\u00a0together, these programs should\u00a0establish\u00a0a reliable, nationwide baseline of support, yet today they fall far short of what families need and data-contrast=\"auto\">\u00a0highlights the\u00a0full range of federally proposed LTSS solutions over the past 30 years.\u00a0This\u00a0gives policymakers\u00a0a clearer view of what has been proposed, what has been tried, and how existing ideas can be\u00a0improved upon\u00a0to create\u00a0a more accessible and affordable long-term care system.\u00a0<\/span>\r\n\r\n<span
