They reveal priorities\u00a0and\u00a0signal\u00a0what\u00a0and whom\u00a0we\u00a0value\u00a0enough to protect when resources tighten.<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Viewed through this lens,\u00a0this Revise conveys several troubling assumptions:<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n<ul>\r\n \t<li><span data-contrast=\"auto\">That\u00a0staying home is optional\u00a0-\u00a0access to\u00a0home- and community-based services (HCBS)\u00a0through Medi-Cal is low priority,\u00a0rather than essential\u00a0to helping\u00a0Californians\u00a0remain safely in their homes and communities<\/span><span data-ccp-props=\"{&quot;335559685&quot;:630,&quot;335559739&quot;:0,&quot;335559991&quot;:270}\">\u00a0<\/span><\/li>\r\n \t<li><span data-contrast=\"auto\">That community-based organizations (CBOs) can continue to absorb\u00a0increasing\u00a0demand with fewer resources<\/span><span data-ccp-props=\"{&quot;335559685&quot;:630,&quot;335559739&quot;:0,&quot;335559991&quot;:270}\">\u00a0<\/span><\/li>\r\n \t<li><span data-contrast=\"auto\">That demographic growth requires no durable public investment<\/span><span data-ccp-props=\"{&quot;335559685&quot;:630,&quot;335559739&quot;:0,&quot;335559991&quot;:270}\">\u00a0<\/span><\/li>\r\n<\/ul>\r\n<span data-contrast=\"auto\">HCBS are not \u201cnice-to-have\" <\/span><span data-contrast=\"auto\">services. <\/span><span data-contrast=\"auto\">They\u2019re needed infrastructure that keep people in their homes and communities. During a <\/span><a href=\"https:\/\/www.assembly.ca.gov\/media\/joint-hearing-budget-subcommittee-no-2-human-services-and-budget-subcommittee-no-1-health-20260325\"><span data-contrast=\"none\">recent\u00a0Assembly hearing<\/span><\/a><span data-contrast=\"auto\"> on proposed reductions to the In-Home Supportive\u00a0<\/span><span data-contrast=\"auto\">Services\u00a0(IHSS)\u00a0program,\u00a0one advocate described\u00a0the program\u00a0plainly:\u00a0\u201cIt\u2019s\u00a0a basic survival need.\u201d\u00a0<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">Reductions\u00a0to\u00a0Medi-Cal will\u00a0impact\u00a0access to\u00a0HCBS\u00a0and\u00a0are harmful to\u00a0older adults and people with disabilities.\u00a0They are\u00a0fiscally counterproductive and inconsistent with California\u2019s\u00a0commitment to building\u00a0a\u00a0\u201cCalifornia for all.\u201d<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<b><span data-contrast=\"auto\">The Hidden Stakes of the May Revise<\/span><\/b><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">The May Revise includes no new investments for older adults and people with disabilities, while\u00a0reducing\u00a0access to Medi-Cal, which will\u00a0decrease\u00a0access to HCBS.\u00a0The most glaring of reductions is the reinstatement of the Medi-Cal asset test\u00a0at $2,000 for\u00a0individuals\/$3,000 for couples.\u00a0Over\u00a060,000 Medi-Cal enrollees are\u00a0anticipated\u00a0to lose coverage\u00a0due to this reduction.<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">California\u2019s older adult population is\u00a0<\/span><a href=\"https:\/\/www.ppic.org\/publication\/californias-aging-population\/\"><span data-contrast=\"none\">projected to grow by nearly 60 percent in less than fifteen years\u2014and when nearly one million Californians are expected to require help with self-care<\/span><\/a><span data-contrast=\"auto\">\u2014\u00a0so\u00a0this\u00a0budgetary\u00a0approach is especially concerning.<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">The\u00a0implications\u00a0will have\u00a0system-wide effects, including:<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n<ul>\r\n \t<li><span data-contrast=\"auto\">Delayed care\u00a0that leads to\u00a0higher acuity and increased costs<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span><\/li>\r\n \t<li><span data-contrast=\"auto\">Greater risk of unnecessary institutionalization<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span><\/li>\r\n \t<li><span data-contrast=\"auto\">Increased strain on family caregivers<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span><\/li>\r\n \t<li><span data-contrast=\"auto\">And\u00a0mounting\u00a0pressure on local governments and\u00a0CBOs\u00a0already\u00a0stretched\u00a0to capacity<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span><\/li>\r\n<\/ul>\r\n<span data-contrast=\"auto\">California is retreating from HCBS at the <\/span><span data-contrast=\"auto\">very <\/span><span data-contrast=\"auto\">moment <\/span><span data-contrast=\"auto\">federal instability threatens access. <\/span><a href=\"https:\/\/www.thescanfoundation.org\/es\/recurso\/impact-of-h-r-1-on-the-health-and-wellbeing-of-older-adults-and-people-with-disabilities-in-california\/\"><span data-contrast=\"none\">H.R. 1\u00a0(2025-26)<\/span><\/a><span data-contrast=\"auto\">\u00a0reduces\u00a0funding for Medi-Cal,\u00a0the primary payer of HCBS.\u00a0Additionally,\u00a0the\u00a0Centers for\u00a0Medicare and\u00a0Medicaid have\u00a0issued a\u00a0<\/span><a href=\"https:\/\/www.dhcs.ca.gov\/Program-Integrity\/Documents\/Deferral-Letter-CA-Q1-2026.pdf\"><span data-contrast=\"none\">letter to\u00a0the state<\/span><\/a><span data-contrast=\"auto\">, warning\u00a0they\u00a0will\u00a0withhold\u00a0$1.3\u00a0billion\u00a0in\u00a0Medi-Cal funds,\u00a0most of\u00a0which are\u00a0earmarked for the\u00a0IHSS\u00a0program.\u00a0Together,\u00a0these impacts\u00a0will\u00a0disproportionately harm\u00a0low-income older adults, people with disabilities, and family caregivers.<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">The evidence is\u00a0already\u00a0clear:\u00a0<\/span><a href=\"https:\/\/www.chcf.org\/wp-content\/uploads\/2025\/10\/HowCutsMediCalHomeCommunityBasedServicesImpactCA.pdf\"><span data-contrast=\"none\">restricting access to HCBS does not save money<\/span><\/a><span data-contrast=\"auto\">. These proposals are short-sighted,\u00a0and\u00a0they undermine years of progress under the Master Plan for Aging and the state\u2019s commitment to rebalancing Medi-Cal spending toward\u00a0HCBS.<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<b><span data-contrast=\"auto\">A State Defined by What It Chooses to Fund<\/span><\/b><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">California\u00a0faces\u00a0a\u00a0budget\u00a0crisis,\u00a0and\u00a0lawmakers are\u00a0being\u00a0asked\u00a0to make\u00a0difficult\u00a0choices. Nevertheless,\u00a0these\u00a0choices\u00a0will carry\u00a0long-term\u00a0human and fiscal costs.<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">State leadership should be focused on long-term sustainability, especially as federal funding grows more uncertain.\u00a0<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">This\u00a0requires more than\u00a0managing\u00a0deficits\u00a0each\u00a0year.\u00a0As the gubernatorial race takes shape, candidates are increasingly pressed to articulate how they would make California more affordable, particularly with respect to HCBS. This moment calls for strong leadership willing to explore new revenue solutions to address the state\u2019s structural deficit and rising cost-of-living challenges.<\/span><span data-ccp-props=\"{&quot;335559739&quot;:0}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">This\u00a0current\u00a0period may be remembered as a turning point;\u00a0not because of one decision, but because of a series of incremental choices that accumulated into lasting consequences.\u00a0<\/span><span data-ccp-props=\"{&quot;201341983&quot;:0,&quot;335559739&quot;:0,&quot;335559740&quot;:276}\">\u00a0<\/span>\r\n\r\n<span data-contrast=\"auto\">The true cost will not be measured on paper, but <\/span><span data-contrast=\"auto\">in <\/span><span data-contrast=\"auto\">Californians\u2019 lives.
