This opportunity strengthens TSF’s approach to disability inclusion by supporting grassroots, lived-experience–led advocacy. The Fund’s participatory model, network of aging and disability leaders, and alignment with narrative and policy change reinforce TSF’s focus on elevating voices often excluded from policymaking.
This project will deepen TSF’s engagement in philanthropy’s evolving efforts to elevate underrepresented voices. The structure and convening power of the Forum and President’s Council create valuable opportunities for shared learning and expanded access to new partners and networks.
CHW Planning Grant and Pilot Within CDA’s Cal Community Connect Initiative
Community Health Workers (CHWs) provide vital health care and service navigation support that the aging and disability network could better leverage with deeper understanding of how to utilize and bill for the CHW services. With support from the MPA Fund, Futuro Health will pilot a sustainable CHW model in collaboration with the California Department of Aging’s Cal Community Connect initiative. At the conclusion of the project, Futuro Health will share an implementation toolkit for replication among the aging and disability network.
Long-Term Care Financing Policy Brief
There are newly emerging policy conversations about federal LTSS/LTCF innovation, both amongst legislative staff and within various aging and health care stakeholder coalitions. This project, which will result in a succinct policy brief on LTSS reform that will be disseminated to key federal policy staff, will support TSF’s larger strategic goal to elevate LTCF reform as an important policy and political issue that needs solutions urgently.
HCBS Definition and Consensus
A shared language is essential to advancing effective home and community-based services (HCBS) policies, but the challenges go beyond terminology. Health Affairs, Forefront, will publish a series of articles in support of disseminating the HCBS definition developed by HRSI.

Sharing the stories of people with lived experience provides an opportunity to highlight what they need to live well at home and in community. With co-funding from Archstone Foundation, The John A Hartford Foundation, and RRF Foundation on Aging, this will support a national summit as part of a national movement to change how society thinks and talks about aging in order to change the systems that shape how we all experience aging.
National Center to Reframe Aging – Driving Toward Sustainability
Sharing the stories of people with lived experience provides an opportunity to highlight what they need to live well at home and in community. With co-funding from Archstone Foundation, The John A Hartford Foundation, and RRF Foundation on Aging, this will continue developing new programs and partnerships as they complete the transition to a self-sustaining central hub to carry the movement to reframe aging forward. This long-term endeavor is building a national movement to change how society thinks and talks about aging in order to change the systems that shape how we all experience aging.
ACO “Near Dual” Research Project
Aging Forward Coalition Los Angeles (AFCLA) – Phase 1 Planning
Regional coalitions in CA are the cornerstone to driving policy and systems change locally and in the state. With co-funding from The Archstone Foundation this project is to determine the feasibility of building a Los Angeles based regional coalition and sustainability plan. This will be accomplished through meetings with stakeholders and people with lived experience.
Planning Grant: Building a Home- and Community-Based Services (HCBS) Impacts Tracker and Information Clearinghouse
Closing the data gap on the impact of cuts as a result of H.R. 1 is critical to understanding the impact on older adults. This planning grant to develop a HCBS Impacts Tracker and Information Clearinghouse will focus on determining how to obtain timely and reliable information about state policy responses to One Big Beautiful Bill Act and their impacts on people who use home and community-based services from multiple sources, including state-based advocates on the ground, national organizations with state-based networks, federal and state Medicaid data sources, and the media.
JPM 2026 – Annual Future of Medicaid Innovation Forum (2)
Ensuring the voices of people with lived experience are at the center of all discussions ensures programs and services meet their needs. Funding for this forum includes the stories of people with lived experience as part of a larger discussion on Medicaid innovation.
Storytelling for ECO Groups
Supporting older adults to be at the center of identifying issues and co-designing solutions to independence at home and in the community is a cornerstone of systems changes. With co-funding from the California Health Care Foundation and Metta Fund, four California-based organizations will support Equity Community Organizing (ECO) Groups. This project provides training and technical assistance to the ECO Groups in support of developing and telling stories to inspire action.
