Pear Suite’s AI-enabled platform empowers community health workers (CHWs), community-based organizations (CBOs), and health plans to deliver high-quality care, measure outcomes, and help make care sustainable and scalable. This investment will support Pear Suite in continuing to transform the role of CHWs in the healthcare system, who serve as intermediaries between health plans and Medicaid members, older adults, people with disabilities, and other communities that have historically faced systemic challenges in accessing care.
Advancing Health Equity in Aging: Discovery Project to Build Alliances and The Opportunity to Organize Older Adults
This project will document and visualize the full landscape of organizing actors, categories, and roles of different sectors to organize older adults. It will learn directly from people working in adjacent spaces, including funders, organizers, academics, and older adults themselves, and develop and prioritize ideas for ways to make the most impact.
