The report, Leveraging COVID-19 Public Health Emergency Flexibilities to Advance Person-Centered Care for Older Adults and People with Complex Care Needs: A Roadmap for Policymakers, was developed in partnership with the Alliance for Health Policy, ATI Advisory, Health Management Associates, and Manatt Health. This Playbook is intended to be a resource for state Medicaid agencies seeking to partner with Community Care Hubs (CCHs) in the design and implementation of social determinants of health (SDOH) initiatives. The Playbook offers an introduction to the key functions of CCHs, and practical advice on how state Medicaid agencies can collaborate with CCHs to: Engage Communities; Expand Community Capacity; Support Operational Excellence; Create Pathways to Financial Sustainability; and Look Upstream. This toolkit is intended to serve as a resource for multidisciplinary care teams, including physicians, nurses, physician assistants, medical assistants, social workers, chaplains, nurse navigators, community health workers, and trained volunteers. It provides actionable steps and guidance to ensure that every older adult’s health outcome goals and care preferences are understood, documented, and integrated into their care by the entire health care team.
